New patients are still welcome although, there may be a wait. We accept most insurance carriers. Please call the office for any insurance questions and to make an appointment. Uninsured patients are also accepted, please call for visit fees. Payment for all co-pay, goods, and services are required at the time of your visit.

Acceptable forms of Payment include: Cash, Personal Checks, MasterCard, Visa and Debit cards. There is a $25.00 fee for returned checks.

Kindly give 24 hours notice to cancel or reschedule an appointment. There is a $50.00 fee for canceling with less than 24 hour notice or for missed appointments. Mahalo.
